CODE OF CONDUCT/WAIVER AGREEMENT
Contribute in every way possible to the enjoyment of the game on and off the field.
Accept the decision of the officials with grace and support them on and off the field.
Be supportive of my team members on and off the field.
Do not create situations that may risk injuring myself and/or other players.

Failure to participate within the conduct listed above or failure to follow the rules and regulations adopted by FIFA, USSF, or the California Soccer Park may result in immediate suspension or dismissal from the league without refund of fees.

AGREEMENT AND RELEASE FROM LIABILITY
I acknowledge that I have voluntarily applied to play soccer at the California Soccer Park in the City of Redding, CA. I am aware that soccer is a hazardous activity. I am voluntarily participating in these activities with knowledge of the danger involved, I hereby agree to accept any and all risks of injury or death, and verify this statement by placing my signature listed below.
As consideration for being permitted by the California Soccer Park at the Park I agree that I, my assignee's, heirs, distributes, guardians, and legal representatives will not make claim against, sue, or attach the property of the Shasta Regional Soccer Association, dba California Soccer Park, or the City of Redding, for injury or damage resulting from negligence or other acts, howsoever caused, by any employee, agent, or contractor of the California Soccer Park or the City of Redding or any of its affiliated organizations as a result of my participation in soccer.  I hereby release the Shasta Regional Soccer Association, California Soccer Park or the City of Redding from all actions, claims, or demands that I, my assignee's, heirs, distributes, guardians, and legal representatives now have or may have hereafter for injury or damage resulting from my participation in soccer.

I have read this agreement and fully understand its contents.  I am aware that this is a release of liability and a contract between myself and the Shasta Regional Soccer Association, dba the California Soccer Park, and/or its affiliated organizations and sign it of my own free will (parent or guardian signature required if a minor).
